Job Description
POSITION SUMMARY
The person in this position supervises the implementation of all treatment to the youth on their caseload.
This position reports to the Program Supervisor.
ESSENTIAL FUNCTIONS:
The Case Manager meets regularly with the client and family, with the client to meet community
service and restitution obligations, educational and employment goals as applicable. The Case
Manager also prepares client records and reports, maintains client scheduling and day-to-day
operations of Case Management services and assists with providing compliance data.
Provides direct services to youth referred to Delaware Home Quest who require Pre-Trial Case
Management.
Specific job-related activities include:
Close enforcement of the conditions of bail dispositions: Vision Quest Case Manager will
report a minimum of once per week to DYRS.
A minimum of weekly face-to-face contact (either at school or at home)
Daily curfew calls
Random curfew checks
School attendance monitoring . Contact with the parent/guardian on a weekly basis
Crisis intervention
In-home intervention
The Case Manager will be present in court with youth to make recommendations for sentencing in
consultation with a DYRS representative.
Other responsibilities include: Responsible for holding youth and families accountable for
completing required services.
Supports parents by understanding the day-to-day challenges of parenting a child with emotional
behavioral needs and involvement with the courts.
Helps translate for parents who are unfamiliar or uncomfortable with system jargon.
Advocates for working within the system, educate professionals with whom they work, provide
information to parents, listen to parents, and help parents build the confidence and skills they need
to advocate for their own children.
Provides detailed documentation regarding the weekly family/individual meetings and/or phone
contacts that occur with each family/youth, probation officer, and other outside agencies. All services
performed must be put into Vision Quest's Care4 database within two (2) business days of service
completion.
Directs the family to proper emergency services, such as the police or crisis hotline. In the event
that a crisis occurs, the Pre Trial Case Manager must also contact the Probation Officer, and Vision
Quest administrators.
Administers and adheres to all company policies and procedures and state regulations for staff
and youth.

EDUCATION & EXPERIENCE:
Bachelor's degree in sociology, psychology, counseling, criminal justice, education, social work,
human services, divinity, rehabilitative services, health services, public health administration or any
related field
1 year experience in related human service work.
QUALIFICATIONS:
Must be 21 years of age or older.
Cleared tuberculosis test results.
Cleared background checks from appropriate entities.
Must have a valid driver’s license and be eligible to drive to facilitate program services as
required.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
May lift and carry up to 10 pounds frequently, up to 20 pounds occasionally, up to 50 pounds
infrequently.
Performs tasks that may involve standing or walking 50% of the day; sitting 50%of the day.
May bend, stoop, reach and grasp as necessary.
