Responsibilities:
The Clinical Support Specialist Lead handles telephone inquiries from patients, families, other physicians and other healthcare professionals relative to patient care questions or needs. As a Clinical Support Specialist Lead you will:
Function as the team lead under the direction of the CSS Managers and Director
Develop, coordinate and implement CSS work schedules and daily assignments
Identify and problem solve technical and workflow issues
Receive inquiries from patient/caregiver and interpret/document all conversations/actions/outcomes in the patient chart
Communicate medical need for patient visits and coordinate with VPA physician on follow up actions
Assess patient needs and communicate method of resolution
Provide on-site coordination of home health documentation
Qualifications:
Qualifications
Emergency Medical Technician (EMT), Licensed Practical Nurse (LPN), or Certified Medical Assistant
5+ years of geriatric clinical experience
5+ years of health care customer service experience
Familiar with home health care referral criteria and services
Proficient in Microsoft Office
Knowledge of compliance (including clinical records) of all local, state and federal laws regarding licensure and certification with Home Health Care compliance standards
Proficient in Microsoft Office
