Job Description
We are expanding our Clinical Operations Team! The Care Coordinator reports to the Manager of Care Management. The Care Coordinator is a key member of the multi-disciplinary care management team. We use a team-based approach to support members to achieve optimal health outcomes, access appropriate care and reduce unnecessary healthcare costs. The Care Coordinator works in collaboration and in continuous partnership with the Registered Nurse-Care Manager (RN CM) and will assist patients and their family/caregivers(s) to help navigate care and manage their complex health conditions.
WHAT YOU WILL BE DOING:
The Care Coordinator will:
Serve as the initial point of contact to engage members to enroll in care management programs and clinical services.
Perform tasks as delegated by the care management nurse to include monitoring patient progress, perform follow-up calls, and identify situations that require action by the patient’s care management team and other health care providers.
Deliver person-centered care coordination, including working with a patient in a way that follows their needs and preferences, increasing patients’ ability for self-management and shared decision-making and increasing health literacy through culturally and linguistically appropriate education.
Track and support patients when they obtain services outside the practice such as Emergency Departments, Hospitals, and other health care facilities.
Provide relevant self-management education and support for patients with chronic illnesses.
Assist patients in problem-solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance).
Assist with data collection and generation of patient summary reports.
Address gaps in care and assist patients in setting up healthcare appointments to close care gaps.
Remain current on trends and industry knowledge establishing self as subject matter expert.
Other duties as required.
WHAT DO YOU NEED
Must have a minimum of 2 years of direct patient care addressing complex/chronic health conditions in a medical practice, community health facility or in a managed care setting.
Associate or bachelor’s degree in healthcare related field or equivalent professional experience, required.
Licensed Practical Nurse, Medical Assistant, or other health related certification, preferred.
Care coordination, navigation, advocacy, or care management experience ideal.
Competency in utilizing technology to communicate and document clinical activities.
Proficient in utilizing Microsoft Outlook, Teams, and SharePoint.
WORK REQUIREMENTS:
