Job Description
We are seeking certified a Certified Medical Assistant to join our team as Care Management Associate. As a Care Management Associate, you will focus on supporting Clinicians and engaging with patients to ensure the provision of high-quality, outcome-based care.
The successful candidate will be a multi-faceted CMA with a strong outcomes orientation, strong problem-solving skills a bias for action, and a high degree of professionalism.
Responsibilities:
Obtains and verifies patient information, history and documents by interviewing patient; recording medical history; confirming purpose of visit
Give instructions to patients as instructed by physicians, Nurse Practitioners, and/or Physician Assistants
Ensure all related reports, labs and information is filed and available in patients’ medical records prior to their appointment
Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls; Gathers required details to inform treating Clinician of ongoing complaints.
Answers incoming telephone calls; responds to patient (and caregiver) questions regarding current complaints, medications, diagnostic testing, orders, treatment, and care plans, as instructed by Clinician
Advises provider of patient's condition, complaints, compliance, or adverse reactions to medication and/or treatment to medication and/or treatment.
Transmit messages from patients and front office staff to physicians, advanced practicing nurses, and physician assistants
Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, oxygen, cold sterilization fluid change, etc.)
Coordinates telephone requests to refill prescription drug medications in accordance with provider's directions and protocols.
Coordinates with scheduling to facilitate required patient visits as appropriate.
Notifies patient of test results as directed by treating provider.
Performs patient outreach, education, and teaching to improve compliance with care plans, under the direction of a provider.
Tracks manages and expedites referrals, consults, and orders to ensure receipt and completion. Monitors status of orders e.g. labs, DME, HHC, Imaging studies, RX, etc.
Supports the completion of medical documents requested by third-party payors and other providers, as directed by clinical providers
Engages with other providers to coordinate patient care and foster compliance with the plan of care, as directed by the Clinician.
Phlebotomy experience/certification is a plus
Qualifications:
Certified Medical Assistant 2+ years experience
Deep commitment to providing complete care
Ability to build rapport with patients
Ability to provide a high level of compassionate patient service
Excellent administrative, organizational and verbal communication skills
Multi-lingual a plus, especially Spanish
Salary & Benefits:
New Hire Experience-Based Salary Range: $35,000 to $44,000
