SUMMARY
The Building Bridges Project will prevent suicide and suicide attempts among American Indian/Alaska Native (AI/AN) adults age 25 and older to reduce the overall suicide rate and the number of suicides in the Fairbanks North Star Borough (FNSB). Building Bridges will build the systems to culturally bridge the service gaps to integrate critical components of the National Strategy for Suicide Prevention and the Tribal Behavioral Health Agenda.
The candidate will be responsible for providing case management service for consumers in the Building Bridges Project. The position assists consumers in learning coping skills and creating stability in their lives. This position is part of a collaborative treatment team and serves as a liaison between consumers and clinical staff.
JOB DUTIES
Develops and maintains individualized care plans for consumers, including consumers transitioning from emergency care and consumers screened at high risk
Maintains thorough knowledge of the Building Bridges Project, as well as an understanding of how this project impacts the other entities of contact
Conducts case management to include monthly tracking, scheduling, and data entry
Enters accurate and timely data into the Electronic Health Record (EHR)
Develops and fosters relationships within the community, stakeholders, and other entities
Refers consumers to support services in the System of Care (SOC)
Attends initial contact meeting with SOC agency to assist with entry paperwork
Builds and maintains an extensive database of community resources to match consumers with eligible resources
Coordinates directly with treatment providers to track and ensure follow-up services are being provided
Tracks and documents the progress of referrals to ensure follow-up
Responsible for providing culturally relevant and trauma-informed service
Promptly responds to incidents and other events as necessary
Acts as a positive representation of FNA to the community
Performs other job-related duties as assigned

Knowledge of substance abuse and mental health; and appropriate skills in working with consumers with Mental Health/Substance Use disorders
Skill in working successfully with the Alaska Native/American Indian population and experience providing culturally specific programming; understanding of and sensitivity to diverse cultures and lifestyles
Skill in managing multiple, concurrent priorities and meeting deadlines
Skill in computer usage

MINIMUM QUALIFICATIONS (Education & Experience)

Bachelor’s degree in public health or related field

SUMMARY
The Suicide Prevention, Intervention, and Postvention (SPIP) Specialist for All Together is responsible for providing comprehensive, culturally appropriate behavioral health services that will reduce the prevalence of suicide among American Indians and Alaska Natives age 14 and older at Effie Kokrine Charter School (EKCS). The methods of Evidence Based Projects (EBP) are to include Natural Helpers, Question – Persuade – Refer (QPR), Counseling on Access to Lethal Means (CALM), and Model Adolescent Suicide Prevention Program.
JOB DUTIES
Responsible for timely implementation of prevention services following evidence-based practices.
Conducts community-based training for SPIP at a minimum of four (4) times a year.
Continuously updates skills through training targeted in Staff Development Plan.
Oversee and train Suicide Response Team.
Develops Community Suicide Response Plan.
Develops suicide protocols and/ or practices with the Effie Kokrine Charter School.
Assists in planning and organizing training opportunities.
Assists coordination with the Interagency Transition Council (ITC) and support the ITC Suicide Prevention Subcommittee.
Attends and participates in meetings, quality improvement activities, and data driven decision making processes to assess project efficacy as required.
Furthers the mission of the organization through active support of the strategic goals and deliverables of the project.
Ensures adherence to data collection requirements.
Performs other job-related duties as assigned.

MINIMUM QUALIFICATIONS (Education & Experience)
Bachelor’s degree in behavioral health or related field. FNA will consider an equivalent combination of relevant education and/ or experience in substitution of the education requirement.
Three (3) years of experience working with Alaska Native/American Indian population and providing culturally specific services.
Three (3) years of experience working in suicide prevention.
Three (3) years of experience working with the youth and young populations.
PREFERRED QUALIFICATIONS (Education & Experience)
In addition to the minimum qualifications:
Master’s degree in behavioral health or related field.
WORKING ENVIRONMENT

This position corresponds to medium work, involving physical exertion of 20-50 lbs. frequently; such exertion involves the lifting and/or moving of young children in the project, and/or goods and materials within the facility. There is frequent walking, standing, climbing, stooping, bending, and squatting/crouching. The majority of work is performed in a classroom setting with a wide variety of people with differing functions, personalities, and abilities. The office is a non-smoking facility with outside smoking areas. May involve local travel between various project components of FNA and home visits where exposure to extreme weather conditions are likely. Works in an environment where protection of client confidentiality is essential. Position requires cleaning the class area every day, including putting supplies in proper space, cleaning and sanitizing surfaces with bleach solution to disinfect. Personal Protective Equipment will be provided where/when needed.

Position receives direct supervision from the Project Manager; however, incumbent is expected to effectively perform day-to-day activity as denoted within job description. Work will be monitored through weekly meetings, monthly reports, and regular communication with your supervisor. General guidelines for operations are based on project requirements and the general policies and procedures of FNA.
